Haha, no problem. I'm always happy to answer questions that stem from curiosity rather than the general shock, confusion and disgust that they normally do.
Ahhh, I thought you meant the piercing size itself, not a reference to what I previously said. That being said I have more than one piercing and not all of them fall into each category. My septum falls beneath one, three, and sorta kinda four.
I wouldn't so much call it extreme piercing but body modification as a whole.
People get modified for one or several of four reasons:
1. Aesthetic
2. Sexual Enhancement
3. Shock Value
4. Spirituality
I ascribe to all of the above. I see body modification as a way of changing one's physical appearance to the way they see fit.
Judging by your question about it triggering an allergic reaction, I can tell you're not entirely sure what scarification entails. There are numerous forms of scarification, but the two most common are Scalpeling/ Cutting and Skin removal.
Scalpeling/ Cutting: consists of drawing a predefined design on the skin via usage of a surgical blade or scalpel.
Skin Removal: (or "Skin Peeling" or "Skinning") is a form of scarification consisting of the removal of areas of skin (rather than just making a cut).
Scarification is something I'd consider an "extreme" body mod (along with things like subdermal implants, large gauge piercings, bifurcations, etc) just because of the pain and risk entailed. It just so happens that I tend to be more interested in them than less extreme mods such as the average lip, eyebrow, navel, etc.
Haha, probably when I get to my goal sizes. Other than that, I'm not too keen on tattoos because I have severe allergies (so I don't have any). I am however in the works of planning a large scale scarification piece. And last but not least, sadly my septum mustache is broken and out of frugality I refuse to let myself buy another until I'm a 0g.

There's a 2g/ 6.5mm (this refers to the diameter) hole in my septum.
I can fit jewelry that's essentially the same circumference as a rounded pencil.
